Official Deazzle “Green My Routine” Contest Rules

HOW TO ENTER the Deazzle “Green My Routine” Contest

NO PURCHASE NECESSARY. PURCHASE WILL NOT INCREASE CHANCES OF WINNING. You can enter this contest in one of two ways: online (complete form on our website or via email) and through the mail. To enter online, complete the “Green My Routine” form or send an email to EarthDay@Deazzle.com. Your email should include your name and your answer to the following question: “What do you pledge to do to live a “greener” life?” To enter through the mail, print your name, and your answer to the following question: “What do you pledge to do to live a “greener” life?” Also include your complete address, email address, age, and telephone number on a postcard and mail it first-class to: Deazzle, 9140 Corsea del Fontana Way, Naples, FL 34109. To be eligible, the email must be registered to Receive the Daily Deazzle emails. Incomplete responses will be disqualified. All responses shall become part of Deazzle’s database. Deazzle LLC is not responsible for lost, misdirected or delayed responses. Responses received by telephone or fax, courier or personal delivery will not be accepted.

CONTEST RULES:

1. The contest is open to U.S. citizens at least 18 years or older. Identification may be required upon request. Management, employees and immediate family members of Deazzle LLC and its affiliates Preferred Medical Group of Companies are prohibited from winning any prizes awarded by Deazzle LLC.

2. The contest begins at 12:00am EST on April 1, 2010 and ends at 11:59pm EST on May 31, 2010 (“Contest Period”). Contest Submissions (as described below) must be submitted between 12:00am EST on April 1, 2010 and 11:59pm EST on April 22, 2010 (“Submission Period”). Selection of the top submissions (to be determined by Deazzle representatives and posted on www.Deazzle.com for voting) starts at 12:00am EST on April 23, 2010 and ends at 11:59pm EST on April 30, 2010 (“Selection Period”). Voting for the contest winner (as described below) begins at 12:00am EST on May 1, 2010 and ends at 11:59pm EST on May 31, 2010 (“Voting Period”).

3. To enter the contest, submit a short response (500 words or less) to the following question: “What do you pledge to do to live a “greener” life?” (each response is a “Submission”) during the Submission Period. Submissions may include text only and may not include any pictures, video or other media. Only entries submitted in accordance with these Official Rules will be accepted. There are two ways to enter your Submission: online (complete form or via email) and through the mail. To enter online, send an email to EarthDay@Deazzle.com. Deazzle LLC is not responsible for lost, misdirected or delayed registrations. Registrations received by telephone or fax, courier or personal delivery will not be accepted. The top submissions will be posted on www.Deazzle.com for the purpose of voting.

4. To be eligible, all Submissions must be received by the end of the Submission Period. After submitting the required information as indicated by these Official Rules you will receive one entry into the contest. LIMIT ONE ENTRY PER PERSON.

5. All eligible entries received during the Submission Period will be entered into this contest. After the Submission Period, Deazzle representatives will select the top submissions to post on www.Deazzle.com for public viewing and voting. The winner of the contest will be determined by public voting; the submission with the most votes at the end of the Voting Period will be the winner. If there is a tie, the submission that was submitted first will be the winner. Only the first vote per email address registered at www.Deazzle.com will be accepted.

6. The prize is not redeemable in cash and must be accepted as awarded.

7. Decisions of the contest judges are final – no substitutions will be available.

8. Winner of the contest will be contacted at the email address used to submit his/her pledge or by the mailing address supplied on the postcard, and the person receiving and replying to the winner announcement email will be considered the winner unless they specify another person within the reply email as the winner. Deazzle LLC will mail the contest prize to the mailing address supplied by the recipient of the winner announcement email/mail.

9. Claiming of prize requires a response to Deazzle LLC from the winning sender email/mailing address within 15 days of being notified of winning at the email address used to enter. Failure to respond shall mean that the winner forfeits the prize. Deazzle LLC is not required to award elsewhere any prizes forfeited by the chosen winner.

10. By claiming the prize, the winner authorizes the use, without additional compensation of his or her name and/or likeness and/or voice/photograph and municipality of residence for promotion and/or advertising purposes in any manner and in any medium (including without limitation, radio broadcasts, newspapers and other publications and in television or film releases, slides, videotape, distribution over the internet and picture data storage) which Deazzle may deem appropriate.

11. In accepting the prize, the winner acknowledges that Deazzle LLC may not be held liable for any loss, damages or injury associated with accepting or using this prize.

12. Winner is solely responsible for all taxes and/or fees that may be incurred.

13. The prize winner must supply Deazzle LLC with his/her legal name, mailing address, birth date, and daytime and nighttime telephone numbers.

14. Winner must sign a release form as prepared and accepted by Deazzle LLC acknowledging his/her acceptance and understanding of the complete contest rules.

15. Deazzle LLC retains the rights, in its absolute and sole discretion, to make substitutions of equivalent kind or approximate value in the event of the unavailability of any prize or component of the prize for any reason whatsoever.

16. This contest is subject to all federal, provincial and municipal laws.

17. Deazzle LLC reserves the right to withdraw or terminate this contest at any time without prior notice.

18. Approximate value of prize is $199.

19. Prize cannot be used in conjunction with any other promotion or offer and may not be separated and must be accepted as awarded, cash equivalent substitution will not be available.

20. One entry per person.

HOW TO ENTER the Deazzle Raffle Contest

For each person who clicks through your referral link and signs up to become a member of the Deazzle community you’ll receive a referral point (or raffle ticket). The drawing will be March 31, 2010.

When you register, you will receive a referral link. For each person who clicks through your referral link and signs up to become a member of the Deazzle community, you’ll receive a referral point. Each referral point = 1 raffle ticket. The drawing will be March 31, 2010.

HOW DEAZZLE TRACKS THE REFERRALS:

After you complete the online form to become a member of the Deazzle community you’ll receive a welcome email which contains your unique referral link. You can share your unique link to get people to sign up as a member of the Deazzle community.
There are numerous ways you can share your unique link. Send your link in an email or post your link anywhere and everywhere (including Facebook, Twitter, blogs, websites, the possibilities are endless).
When someone clicks your link he/she will be directed to Deazzle’s signup page. Only after a person successfully registers will you get credit for the signup.

CONTEST RULES:

Your total referrals will be increased by 1 for each qualified referral. A qualified referral is a successful registration that utilized your unique link to register on the Deazzle.com website. Only 1 qualified referral point will be awarded per signing up email address. If a person signs up multiple times on the website to obtain the Daily Deazzle in multiple cities, only the first time registering on the site will generate a referral point for you. Automated signups will not be counted towards qualified referrals.

A person can only win one of the drawings. In the event a person’s name is drawn in more than one drawing, he/she will be awarded the prize for the first drawing his/her name was drawn.

The prize is not redeemable in cash and must be accepted as awarded.
